Genomics is a rapidly growing field that has the potential to revolutionize healthcare. In recent years, the emergence of companies such as 23andMe has enabled individuals to gain insight into their own genetic makeup and learn more about their health risks. But is 23andMe worth it?
To answer this question, itβs important to first understand what 23andMe offers. Through a simple saliva test, 23andMe can provide users with reports on more than 125 genetic conditions, including inherited diseases and traits. The company also provides ancestry information and can offer insights into how certain drugs may affect people based on their genetic makeup.
While 23andMe provides valuable information, it is important to note that the data provided is not a substitute for medical advice. The results of genetic tests can be complicated to interpret and should be discussed with a healthcare professional. Additionally, 23andMe does not provide comprehensive coverage of all known genetic conditions, and there is the potential for false positives or false negatives.
The value of 23andMe is ultimately up to the individual. For those who are interested in gaining more insight into their genetic makeup, the cost of the test may be worthwhile. However, itβs important to remember that the results of the test are not a substitute for medical advice and should be discussed with a healthcare professional. Ultimately, 23andMe can provide valuable information, but it is important to understand the limitations before making a decision.
Genetics, diet and weight management
Genetics undoubtedly play a role in weight, as do lifestyle choices such as diet and exercise. While it is true that some people may be predisposed to gaining weight, it is important to note that a healthy diet and regular exercise are still essential for maintaining a healthy weight.
Weight is determined by the number of calories consumed versus the number of calories burned. This is known as the energy balance equation. Genetics determine how efficiently our bodies use energy and store fat, as well as influencing how we respond to the environment and to our diets.
Genetics can influence weight in a number of ways. For example, some people may have a genetic predisposition to obesity due to the hormones leptin and insulin, which regulate appetite and fat storage. Additionally, certain genes may also influence how quickly we burn calories and how much we can eat before feeling full.
Another way genetics can influence weight is by affecting our metabolism. Our metabolism is the rate at which our body converts food into energy, which can affect how quickly or slowly we gain or lose weight. People with a higher metabolic rate tend to burn calories more quickly, while those with a lower metabolic rate tend to burn calories more slowly.
Genetics can also influence our cravings and appetite, as well as how quickly we become full. Some people may have a greater desire for sweet, high-calorie foods, while others may be more likely to crave savory foods. In addition, some people may be able to eat large meals and still feel full, while others may become full much more quickly.
It is important to remember that while genetics do play a role in weight, a healthy diet and regular exercise are still essential for maintaining a healthy weight. Eating a balanced diet, including plenty of fruits, vegetables, and whole grains, can help to ensure that the body is receiving all the nutrients it needs. Additionally, regular physical activity can help to increase the metabolism and burn calories.
Overall, genetics can play a role in weight, but it is important to remember that a healthy diet and regular exercise are still essential for maintaining a healthy weight. Genetics may influence our cravings, metabolism, and appetite, but these can be managed through a balanced diet and regular physical activity. Making healthy choices and staying active can help ensure that your weight is maintained in a healthy way.

Challenges in specimen reception
Specimen reception isn’t the most glamorous area of the laboratory, but it’s one of the most important. An error in booking in or triaging a sample can cause significant delays if it isn’t picked up before a sample is sent down the wrong pathway – something which can lead to loss of sample if the wrong testing is undertaken.
In many laboratories, particularly in genomics, the different pathways can make the sample receipt process complex and confusing. Taking genomics as an example, different tests require different sample types, extractions, cultures and so on. Other times the laboratory may receive DNA from another lab, and in this case the quality control checks needed may differ from internally extracted samples. It is this complexity which demands the attention to detail and scientific expertise of reception staff, and this demonstrates the skilled nature of the work in this section which may be overlooked.
Many labs face challenges with high staff turnover in their reception departments, something which may be caused (at least in part) by the unglamorous reputation of working in these areas. The portrayal of these departments is often as busy, understaffed and chaotic, something which isn’t true in many cases.
Specimen reception needs a make over, and the hardworking staff need to have more responsibility, flexibility and variety in their daily tasks. These up challenges can be addressed by more effective communication and training in reception departments, by putting the importance of accuracy and efficiency forefront in the minds of reception staff.

Pharmacogenomics in cancer
A few years ago, treating cancers with chemotherapy was in effect, a game of Russian roulette. The effects of toxicity to many of the drugs used could be as damaging as the cancer itself, and even cause death in some rare cases.
This is where pharmacogenomics has really started to make an impact on cancer patients treatment – the ability to screen patients before undergoing treatment with chemotherapy drugs such as Capacitabine allows clinicians to alter the treatment of patients at risk of toxicity, through identifying mutations in the DPYD gene (which encodes the enzyme which breaks down these drugs).
The challenge for genomic laboratories has been how to introduce this new service which requires a quick turnaround time (due to the implications on treatment of cancer patients). This has lead to the use of alternative technologies for interrogating the genome for mutations in the DPYD gene rather than the traditional PCR based methods (which are time consuming and labour intensive), such as LAMP testing (the same technology utilized in the COVID pandemic) and mass spectrometry.
The high throughput and speediness of these techniques has so far allowed laboratories to keep up with the demand for this new service, however as more pharmacogenomic testing is undertaken in medical genetics laboratories, more capacity will be needed to help meet the needs of patients.
